  1. Complete a Business Associate Agreement (BAA) as a one-time submission
  2. For each patient appropriate for treatment with SYLVANT®, complete a Benefit Investigation Form (BIF).
  3. Fax the completed forms to 1-855-299-8845

After receiving your completed BIF, Janssen CarePath will research your patient’s health coverage and return a Verification of Benefits (VOB) within 48 hours. Janssen CarePath Support will also attempt to reach your patient via telephone to review benefits and discuss potential cost support options.
